Instructions for creating your own phone-accessible health records document:

  1. Sign in to your Google account, or create one if you don't already have one. This enables you to create Google docs.
  2. If you don't know how to create a Google account, just google "Setting up a Google account." You will find an explanation, it's really easy.
  3. Once you have a Google account, sign into your account and create a Google doc called "Health Records" for each person who would like to have access to his/her records. For example, if you want to create a Health Records document for John, call it "Health Records_John."
  4. Share the document with John.
  5. Type all of John’s immunizations, including dates, into the document.
  6. Type in any other health information you would like to include, such as allergies, medications, health history.
  7. Email John a link to his Health Record Google doc, and tell him the following:
For iPhone users:
  1. Open the email ON YOUR PHONE
  2. Click on the link in the email - the Google doc will open on the phone
  3. Click the Upload icon, which is at the bottom of your phone's home screen, in the middle (a little square with an up-arrow on it)
  4. Click Add to Home Screen
  5. Click Add (upper-right corner of screen)
For Android Users:
  1. Open the email ON YOUR PHONE
  2. Click on the link
  3. Select to open the document in Chrome, not Google docs
  4. Once the doc is open, click on the 3 vertical dots in the upper-right of the screen.
  5. Select "Add to homescreen"
  1. John should now have an icon that takes him to his Health Records Google doc, on his phone's home screen!
